hello, do you have a template of a review form ( 10 year, light touch ) and how to approach completing the form? Many thanks.

Nothing on this board constitutes legal advice - always consult a professional about specific problems

Hello Chris,
Many thanks for getting back to me. It looks nice and short. Should I send extra information i.e. current consultant letters or just say no change? Is it better to send all I can? many thanks

Hi fluffball,
Always best to treat every claim as a new one. I wrote ( no change ) on a claim once on virtually every question, and lost my award.
Any evidence which you feel is beneficial, you can include - if I remember correctly, the only thing they say not to send is future appointment letters.
Hope this helps,
Chris
