Hello,
I'm currently going through the online form for migrating from ESA and Housing Benefit to UC. However, I'm unsure of whether to add that since my last claim of ESA and indeed since any of my last benefit (I get PIP tpo which is up for renewal/reapplication this year) I have got a trapped nerve. On top of my physical and mental health conditions that already limit what I can do on a daily basis I now have a medical condition and I've had it for nine and a half years that is severely Limiting. Could you please help me to know whether I should and how I should add this to the online form or as a journal entry after application?

No there is no need to mention this if you are in the ESA Support Group.
If you are in the ESA WRAG you could mention the trapped nerve in a Journal message to the Work Coach just so they don't try and send you miles to a workshop to create a new CV etc.
