Yes, you can. It is printed on the form to not delay returning it if you are waiting for medical evidence as this can be posted later.

There's nothing to stop you showing your PIP form to your GP, but most GP's have neither the time or the inclination to look at such things. Also many of the questions on the PIP form your GP will have no knowledge or proof. For instance - can you take a shower or bath? A doctor might guess that someone has difficulties because of their condition but generally they will only know the answer from what the patient tells them. That is true of quite a lot of the questions on the PIP form.

If there are particular things that you want a doctor to mention in a letter, I would write those down and give them to them. Now it depends on your doctor, but I have given a letter to a doctor and asked them if they would be willing to write something similar to the DWP. Fortunately, they have always agreed just to take what I've said - but that's because I am accustomed to doing it. If you have a good relationship with your doctor, you can at least discuss what you want them to say.

There is a form that you can take a long to the GP and ask them to fill in. It's under the PIP tab and it's call GP notes.

The majority of assessments these days are telephone. But yes you can phone PIP and tell them what you have described.
