I will probably be phoning the DWP next week to ask them to send me the claim form. Before I do, a further question: is it possible to score points for more than one descriptor within the same activity? For example, the dressing and undressing section includes the following:
Needs assistance to be able to dress or undress their lower body. 2 points.
Needs assistance to be able to dress or undress their upper body. 4 points.
If both of the above apply, do you score points for both? Logically it would seem the answer should be yes if both are true, but I'm not clear on this.

Hi tintack
You will see from the following BenefitsandWork guide that you cannot combine the points within a descriptor as you only get the highest points the once.

Thanks David.
Is it best to still mention all descriptors within the same activity that apply? I'm wondering if it's best to only mention the highest scoring one, in case mentioning lower scoring descriptors in the same activity encourages them to decide the lower scoring one applies; or if it's better to mention everything that applies, so if you don't get points for the higher scoring descriptor you may still get points for the lower scoring one. My gut feeling is the latter is probably the better option in order to give as many chances as possible to score points.

Hi tintack
I would say that your gut feeling is right and it's best to mention everything that applies.
