ESA / UC Work Capability Assessment Evidence to Send?

Hi, I am going to start the process of applying for ESA credits only in the hope of establishing LCWRA for my daughter so payments can be made as soon as she claims UC after leaving college. I'm collating evidence to send but have a question. Her EHCP lists her difficulties quite thoroughly. If the EHCP states she has xxxxx which causes her to xxxxx, which has been taken from the original report, would the EHCP be sufficient or do I need to find the original reports as well? Some of the reports are fairly old although still relevant, hence still being in the EHCP. Thanks
