Periodic Enquiry Form DLA 250

This form is very like the form I first filled in 2003. Do you have to answer all the Questions in it, or can I not answer as "ongoing" to most of them as my health has not changed, I am worse now and on Oxygen can I just add how my health has deteriated.

It is extremely unlikely that the decision maker has access to your 2003 form. We would reccomend that claimants fill in forms on the assumption that the decision maker knows absolutely nothing about their condition and how it affects them, I'm afraid.
