Esa esa50 form and DVLA form

I have mental illness and I have started to fill out this form with my mum. It is exhausting! It has to be in by the 7th of Jan and Im pretty sure I can make that deadline.
The DVLA form has to be in by the 28th Dec, but my question is if I dont have time to get all the evidence and fill in a form what shall I say to them? Shall i just ring them and tell them that I will be claiming it but I will have to ask for another form in the future cos I just cant manage both forms at once! I want to do the ESA form first then DVLA in a couple of months. Only problem is I already asked for the DVLA form 2 weeks ago and it obvouisly has a deadline on it now.
Also, I havent been asigned a CPN yet which is really annoying because I cant put a name down on the form yet & Im worried that will effect my claim.
Any advice would be great.

Hi,
Do I take it you mean a DLA (Disability Living Allowance) form as opposed to a DVLA (Driver and Vehicle Licensing Agency) form?.
If so, re the DLA form if you return it within the time limit by 28th December, if you are awarded DLA, it will be backdated from the date you requested the forms (although there is a maximum time for backdating). If the form is late or you don't send it, you will lose any backdating.
You could always fill it out as much as possible to get it in by the 28th (bearing in mind it will be delayed over Christmas etc) and put down you will send in extra evidence asap. Although you do run the risk of it being dealt with before you get the evidence in. However you maybe able to appeal an adverse decision.
Getting the ESA form in ontime is essential though or your assessment rate payments will stop.

I did mean the DLA form not the DVLA form!!
I am managing to get the ESA form in but I will have to wait until Jan and will then apply for DLA again (new form etc) as I am really too busy over xmas. Im not even at home.

I wonder if you can help me - I am applying for ESA for mental health reasons - do you think it will make a big difference if my CPN fills in the form or it is ok if I fill it out? What do you think.
I have started filling it out but have a cpn appt to go to tomorrow.
Any advice be much appreciated.

Have you looked at the ESA for Mental Health or Learning Dificulties document in the Members area, which explains how to map your disabilities onto the ESA tests?
Your CPN may be able to explain you condition in more technical terms, but you are more likely to be able to explain your condition as it affects you on a day to day basis.
Ultimately the decision may come down to how much time your CPN can spend helping you, rushing to complete it, will probably not help your case.

Yes I have gone through the guide along with my ESA form and typed up answers in a word document to show my cpn tomorrow.
We are both going to go through it tomorrow and I have spent about 8 hours on producing the answers and typing them up!
Hes also going to do me a supporting statement aswell which will be most helpful I hope in my claim.
I have spoken to my doctor aswell and asked him when he does the report to atos whether he can send it to me first and he declined. So I said to him can I send you a copy of my completed form and he agreed yes you can.
Its kinda annoying because my GP doesnt really have anything to do with my care. The mental health team are my primary care. Doc has really no idea whats going on with me let alone write a report on whether im fit for work !
