Hi
I have been through the ESA system I got put on to the WRAG also been to pathways unfortunately I ended up in hospital with another illness on top of the ones I already had.
I was asked to have a reassessment and got 0 points from ATOS so Benefits were stopped.
I have applied to have a tribunal got a letter back to say it wont be until may.
The thing is my Consultants and GP has given me more medication to take relating to the other added illnesses and as I am on the assessment rate I don't get free prescriptions.
I rang up ESA Office and asked them is there any way I can get free prescriptions as I am so many drugs now they suggested I should go onto income related ESA Support instead of Contribution ESA Support.
I received a rather fat form this morning on the front page is said I was on Contribution based ESA but I have asked to go onto income-related ESA support.
The question is by filling in this form will I be denied my tribunal and is there any way they can stop the assessment rate I am getting at the moment and freeze all payments to me so I do not get any money at all.
I do not work have no other part time job as the nature of my health conditions make impossible to hold down any type of work also I have no other income other than the reassessment rate.
Will I be shooting myself in the foot by filling out this form knowing the currant situation of the welfare reform.

call the NHS Formsline on 0845 610 1112
or 0845 610 1112
You will have to fill in an HC1 form 'Claim for Help with Health Costs' giving various details of your circumstances and then send it off in the pre-paid envelope provided.
If you qualify for help you will be sent an HC2 Certificate for full help, or an HC3 Certificate for partial help,

If you don't qualify for an HC2, you can apply for a prescription pre-payment form at the following site.
www.nhsbsa.nhs.uk/1127.aspx
They cost £104/year and entitle you to unlimited prescriptions.

Just one thing the form they have sent me is re-assessment form ESA 10/10 its about 26 pages long definitely not an HC1 or HC2 I am waiting for a tribunal I think they want me to be assessed by Atos again as its a re-assessment form not an HC1 or HC2
