which form?

Hi,

Which form will I get when I cross from IB to ESA, DWP rang me today and said they would be sending one out, but I forgot to ask which form, as I want to look it up and see what sort of questions they ask.

My son has received a form for ESA, and the first question is about is he receiving any ongoing treatment, eg: dialysis, chemo and TPN, the reason I ask is that I am a TPN ( total parenteral nutrition) patient so should be exempt from the WCA and medical. Hope someone can help, and hope I have explained myself properly

I'm not certain but I would suspect it will be the new ESA50 03/11. This is covered by the new ESA guides in the ESA section of the members area.
