The important thing is to make sure it is sent to your local DWP office as identified on any of the correspondance you have received, however, it won't hurt to addess it to the Decision Maker.

Make sure that any additional evidence is securely attached to the ESA50, each page should have your name and NI number included at the top of the page, and of course, make sure you retain a copy before posting.

Yes, send a copy of everything, ESA60 and evidence, to both the DWP and ATOS.
