WCA - which form and help please?

The question I have it that after looking at this fantastic site I am not sure which one I should fill out? New 21 page or Old 28 page - how will I know which one I will be sent?
Am I right to follow the ESA Resources and also incorporate the Employment and Support Allowance claims on physical & mental health grounds: a guide to the work capability assessment?

The current ESA50 is 21 pages long and has "ESA50 03/11" printed on the bottom of the first page.The ME group I am apart of is suggesting that people download the ESA form and start to slowing fill it out on your PC and save it so to be ready for when it arrives - I have been kindly sent the link for the New ESA50. I have been on IB for around 7 years so I will be changing to ESA.

The guides you mentioned are the ones to look at, make sure you use the ones at the top of the page, again dated 03/11.

I too have M.E. and have been on I.B. for about 10yrs.
I have downloaded the ESA50 and am in the process of completing it! Easier said than done though! It's beginning to stress me out! I have until the 15th Sept, so am going to chip away at it bit by bit.
