IS10 claim form

After a recent visit to CAB i have been told to apply for severe disability premium for myself as i have just been awarded DLA at middle rate. My husband gets dla also and has a carer who gets paid carers allowance. CAB rang for the form IS10 and it came today. It came in an envelope which was handwritten in a scrawl. There was no return envelope (which there should have been) and there is no phone number or address where it goes back to. Also there is no where on this form that we actually put our name or address and NI number. If I fill this in as it is they would not know who sent it at all. There is no question on when dla was awarded even. Any ideas?

Could you call cab and explain this to them? They should be able to point you in the right direction where the form goes back to, or get another one sent to you, with a pre paid envleope attached, and a address and phone number who you should send it back to. First point of call is cab and show them what you received. Sounds like someone at dwp messed up somewhere. Lots of luck
