ESA50 Question 15. No "NO" Box was created by george
I am trying to answer ESA50 Question 15.

“Can you leave home on your own and go to places you don’t know?”

I need to answer No to this question.

In your notes on Pg 51 of esa_mental_july_12.pdf you say that if this is the case one should tick no.

However, there is no tick box for “No”. Only for: “Usually”, ”Not Very Often“ and “It Varies”.

What should I do? Should I perhaps draw my own “No” Box onto the ESA50 and then Tick that?

There is nothing to prevent you from striking through the three options as printed at Part 15 of the ESA50 and simply writing "no".

As the guide says;
'Tis the most unfit for purpose form ever!

Follow Crazydiamond's advice and explain why in the box below
or on a separate sheet of paper.

Driven to distraction we gave up on the boxes altogether.
Sent a written statement instead, which Atos accepted.
