hi all, i have completed the dreaded form and am sending it back today, i have added plenty of information about my illness and how it affects me daily, and i have also included a letter from my gp with a diagnosis and medication etc, the letter does say dictated but not signed by my doctor will this make any difference as it is from his secretary. i just want the letter gone and decision reached so i can stop being stressed and move on with my life. happy new year and i will keep you informed of any decision i get, thanks for all the help the site provided.
