Shock horror I've just discovered that I have forgot to put some evidence in my ESA50 form which I posted early December last year. Have a memory problem besides others and we tried to be precise but hay ho I messed up

What I would like to know is should I send them to either Atos or DWP or both or not bother, it is oxygen readings while I am asleep and would prove what I am saying?

Would appreciate some help.

Hi M,

I would send copy to both ATOS, (same address as ESA50 went to ) and FAO The ESA Decision Maker, at the address on any letters from The DWP regarding your ESA.

Put you name, NI Number and DOB at the top of any extra pages, and put in a covering letter stating that you wish this evidence to be taken into account along with your ESA50.

If, in the mean time you receive an appointment for a "medical", you could also take a copy along and hand it to the ATOS HCP before the assessment.

Hi M,

You could mention that you neglected to include this information with your ESA50 as a result of your limitations, if that was indeed the case.

The thing is I can't remember what I did, just found it in the file, so I presume I didn't send it but there again I could have put it in and this could be my copy, so I don't really know.
