first ESA 50 form was created by b muse
I am about to complete my first ESA 50 form and am totally traumatised by it all.. what does medical documents mean and how do I obtain any to send in with my form ..thanks

'Medical documents' refers to letters from GP's, specialists, therapists etc. Some doctors are willing to provide letters to support their patients' claims, some do not as a matter of policy. Some people have CPN's, social workers etc who are prepared to provide such letters.

Many patients ask their consultants to send them a copy of all letters they write about them (to their GP for instance), these can be helpful and will be provided free of charge.

Failing all this, it can be helpful to get a copy of your medical records from your GP and submit a copy of anything that might be helpful. There might be a charge for this

Don't let your hunt for documentation delay your submission of the completed ESA50, it is very important that this is returned on time. You can send supporting evidence in later if need be. Make sure that you use our ESA claim guides to your best advantage.

Thank you for your reply .. I am having problems obtaining copies of letters from psychiatrist etc as GP will not give me any as they are marked should not be disclosed to any other person etc..I have never been sent any copies of letters from CPN or other support workers in the past .. Also I moved out of the area (where I lived for 5 years) and I no longer see CPN or mental health worker am still waiting to see psychiatrist ..am getting worried as I don't seem to have any written evidence to send .. Should I see my GP and ask for help ..

I think this would be a good idea, you can raise the problems you are having accessing your medical records with them at the same time.
