It depends on the benefit, for ESA you can cancel once without fault, for DLA I am not aware of any limit, but would recommend that you stick to only one cancellation. Currently for PIP we are not sure of the situation as the agreed procedures between the DWP and the assessing agencies (ATOS and Capita), are not currently public.
You should be able to cancel over the phone, all of the appointment letters should have a telephone number on them.
As to your reasons for cancelling, I am not aware of a reason being required, but I would not cancel an appointment without having one.
