DLA application form online?

I have been sent a DLA application form as my current DLA is due to run out later this year and I wanted to know if anyone knows anywhere online I can download a DLA form so I can use one to just as a draft kind? Or even just see one online I can just print out?
If anyone has a link to a DLA application or has one saved and can send me one I would appreciate it greatly.

I do not think the renewal form has ever been online, but all of the Adult DLA forms have been pulled by the DWP due to the introduction of PIP.

Not significantly, there should be no problems with you using the DLA Claim guides with a renewal form, do not forget that the DWP will treat it effectively as a new claim, so do not assume that they wil have access to anything you have used before.

DO you think its OK for me to complete on my own? Have you ever known anyone complete a DLA form on their own (ie without the CAB etc) and actually be granted DLA?

Most claimants complete their own forms, only you or someone very close to you will actually know what your care and mobility needs are, although I would recommend you speak to your GP and anybody else who provides you with medical assistance for their input as well.
