They may be able to get one from their local JC+ office, failing that they need to contact ATOS for a replacement, this could take several days, and the four weeks to complete the form is unlikely to be extended.

If they have the facility to print one, then they can use the electronic version

ATOS have no authority to extend the period, he needs to contact the DWP office dealing with his claim, but extensions are very rare and we regularly hear of them being offered and then not recorded by the DWP staff, so any extension has to be in writing.
