Addressing a letter in support of ESA claimant

When a family member, friend, or carer wishes to contribute evidence to includes with an ESA 50 to whom ought should s/he address the lsupporting letter? ATOS, DWP or To Whom It May Concern?

"To Whom it May Concern" should be fine.

I would finish off the statement with something like :

"The above statement is true and accurate to the best of my knowledge

Sign Name :

Print Name :

Date :"

Thanks, bro58 The letter will be included with the ESA 50 and so will be read by ATOS and the DM to whom copies of the form and all supporting data will be be sent.
