Incapacity to ESA support group WITHOUT MEDICAL was created by wesley meredith
I would like to say a big THANK YOU for the help of benefitsandwork to help me fill out the lengthy and tedious ESA form. The pdf guide was invaluable and this, im sure, was the only way i managed to complete the ESA form to enable me to get into the support group of esa. Along with the form i included 46 extra information pages, reports from 7 specialists (even though i had to threaten the complaints procedure to my gp for refusing to write a report (details to follow)) and prescription lists, treatments etc. I also wrote a letter asking for copies of requests for information and copies of all reports received to be sent to me in case of any consequential appeals and/or tribunals arising.
My gp refused to write a medical report citing, "I will not be writing any medical reports for you and will only issue one if requested by the dwp via their own request form. I am unable to sieve through over 20 years of history to write an essay when it may not help with your application. Instead, i will print a copy of your medical summary and you can provide this with your claim form."
I must add, i urge everyone to ask for a printout of the medical summary as i found out conditions i have never known about. For instance, i found out i had heart problems (i was told the medication i was prescribed for migraine was actually treating an underlying heart problem)- the gp will not discuss this and dismisses me when i query the summary. I also found out i had pancreatitis too.
I was so relived when i received the call and it made it better was that the call was made on my birthday lol. I have been awarded it until september 2014, but its a massive weight lifted off my shoulders, even if only for a year or so.
