I gathered as much, hence the letter to reception explaining the pad and also how to do it via NHS Digital.
I've got an email address for the practice manager now so will contact them today.

I'm so sorry that you are having to go through so much. I know it's not appropriate at the moment, but you might want to make a complaint against your doctor in the future. It is not difficult for them to provide the right form.

Agreed, I'm treating it as a complaint now. It's sad to have to do it but they need to know. I had to complain about the hospital mum was in initially as well, it's been a poor show from doctors, unfortunately. Sent off the complaint email over the weekend. Got a reply first thing Monday saying the practice manager is on leave until 7th June but one of the GPs issued the form digitally on the 13th!!!
I thanked them but explained no one had even bothered to tell us, even when I was still asking for it! So that was about 2.5 weeks ago, mum has had no call or post from DWP.
She's pretty much incoherent now, do you think I can ring the DWP to ask where it's all up to? I don't have Power of Attorney as we decided not to bother with it after finding out there's a 6 month wait.

I do feel the anguish you are going through.
Are you able to phone DWP when you are at your mothers house, as they will probably say under GDPR they cannot discuss anything with you, at the same time ask them to make a note that your mother gives permission for them to talk to you.
