Well, a week after dropping the letter off the GP had to go out and see my mum anyway so he was asked about the form. He said it was all in hand and "the DS1500 will be ready after 3pm this afternoon".

Soon as I got the news I was so happy! No more messing about. Went to get it, got in the car, was driving off and it's a DNACPR form, that we already have, which is certainly NOT a DS1500.

My sister said she'd ask the Palliative Care team as they may know but I have officially given up now. It's been over a month and I'm getting nowhere. I'll write to the DWP to update them and if they can do what they want after that tbh. Mum is bedbound in a hospital bed at home and barely eating now. If they want to assess her then they can come out or they can see from her discharge notes that's she's on the GSF register which is for terminally ill people.

I am sorry to hear the news about your mother.
Have you tried contacting Macmillan Cancer Support, they provide excellent advice, not just about benefits, but practical stuff too. It may be worthwhile seeing if they can help liaise with GP and or hospital.

Hi LL. I did ring MacMillan a couple of weeks back but the lady sounded a bit confused and said she wasn't the best person to ask and to ring another time or contact the Palliative Care team which I've tried to do for the past few days (we end up playing phone tag sadly). I'm so busy I didn't think to ring MM back. I may try them tomorrow, it's really upsetting to have to keep constantly ringing and chasing people about this when I 1)hate phone calls, 2) have so many other things to sort out and 3) am caring for my mum, myself and my family and trying to work all at the same time. At least mum has a friend staying with her, if they weren't she'd have to be in a home. No other family (other than my sister coming a few times a month for a couple of hours) are helping aside from hubby and I. (Sorry for the whinge)

Anwyay, I've written out a letter to the DWP now and will send it off regardless as I'm concerned if they find anything out they're going to ask why no one has been in touch in the past 2 months and just stop her money. I'll tell them I'll keep fighting for the form.

My thoughts are with you at this sad moment in time;

I am trying to work out why you were given a DNACPR form and not a DS1500, I can only assume the person who gave you the form was new and did not know the difference.

Where do you get a DS1500 form?

Your doctor, consultant, Macmillan nurse or healthcare professional should have copies and will be able to fill one out for you before sending it to you or directly to the relevant government office. You don’t have to pay for the form.

If someone else is applying for a benefit on your behalf, they may need to ask your doctor or consultant for a DS1500 form for you. They can do this without you knowing they’re applying for benefits under the special rules.

Hi Ravenmiss,

I worked in a doctors surgery, unfortunately the doctors do not know where all the forms come from, it is the practice manager or reception team that can get this for you. It is a green form on a tear off pad and should be found in reception. If you have any problems just speak to the practice manager. Good luck

The GP gave me the form he's not new, I've just no idea why this is such an issue for them.
