Thanks to your advice, I filled it in and phoned the JC+ helpline, explaining the situation that the previous 2 went astray in the post and they happily made an "AJAX" appointment at my local JC+ for later in the afternoon.
I arrived and explained the situation to the kind gentleman who was only too happy to scan it in and send it off electronically to the benefit centre there and then. He showed me his screen to confirm it had been sent.
He handed me the form back and I popped it in the supplied freepost envelope and posted that too, so they have no excuse to say it's gone missing this time!!!! He also told me the JC+ will retain a copy for a month too, as further proof that I'd been and that they'd sent it on my behalf.
Thanks again for your suggestion, as I would have never thought to ask them to scan it and submit it electronically. Let's hope, after all this, there are no further problems!

Hi J,
Yes, it is known as an AJAX/AJACS :
A = Accessing
J = Jobcentre
C = Customer
S = Services
AJCS = Accessing Jobcentre Customer Services.
By appointment only.
Not a lot of people know that !!!!!
