Hello all

Is there anyway I can get hold of the PIP form? I would like to get hold of a generic copy of the form to get a feel for things before reporting a Change of circumstances and being sent a form from them. This is just my idea of being fully prepared. We now have all the evidence, just putting the last few things in place before the "clock starts ticking" so to speak.
