Hi Ob,
If he is not a registered Doctor in medicine, but is entitled to use the title Doctor due to a qualification that he has obtained and is only being referred to as your carer, you should make this clear !!

So I've numbered and included (as well as typed answers to PIP form): 1 Consultant's letters in gate order about my physical condition 2. Same but about mental health condition. 3. My care plan for both conditions. 4 Copies of last 4 months prescription scripts.
I'll keep you updated how it goes.

They'll look at it when it reaches the top of the outstanding pile so if you send it in early it will be nearer the top than if you send it in later.
