I have an address that has come with the form that is ' PIP Post Handling Site B, Wolverhampton. WV99 1AA' Should I get the GP to put this at the top of her letter and put dear whomever it may concern. (obviously the envelope has to have my address so I get the letter sent to me first to add to my claim form)
If you are sending it with your claim form then it doesn't matter too much what address it has on it, but if the GP wants one then the DWP address is as good as any, ultimately it is the Assessment Provider who needs to get a copy, the DWP will include it in the file that they send on.
Don't forget to take a copy of it before you send it.

Not necessarily., I would contact the AP for an address.
