Hello there,
I was on ESA between 2010-2015 when I was found Fit for Work. I am now applying again after depression became worse and I was found to have a new problem- with my ears. Though I have been going to specialists for my ears since last year, and they have told me that I have a problem, they have not told me what the problem is. The DWP have sent me an ESA1 form and told me to get a sick note from my GP. I've never had a Sick Note before and I don't really know what to ask for. Also, will it count against me if I have a problem (the bones in my ears don't work) but I don't have a name for it?

I'm asking for advice at every stage of my ESA application because I've had such a traumatic time since I was taken off it last year - and I'm so scared of messing up the process.

There's no magic words, just explain to your GP why you are unable to work and ask whether they will certify this by issuing a Fit Note.
