Capability for work questionnaire. Help please.

Hello,
I received this form which is confusing me.
6 yrs ago I was diagnosed with breast cancer, had chemo, an op and radiation treatments.
The chemo whilst keeping me alive left me with on goin neurophy both hands and feet.
Basically I'm in remission, not cured but no evidence of cancer right now.

The form asks do you have cancer yes or no. Nowhere to explain anything as I have above.
If I answer no I have to go to other health conditions etc.

If I say yes I don't have to fill in the rest of the form if I don't want to.

What to do? I haven't had a form like this before.

We always tell people not to be limited by the tick boxes. If none of them accurately reflect your situation, write a short sentence to that effect and direct the reader to the supplementary information where you can explain things in detail.

As you are currently in remission and are not actively undergoing treatment I think you will need to complete the form in full in order to give yourself the best chance of securing the award.

Notwithstanding Mrs H's advice, based on your post I think you need to complete the ESA50 in full and in detail.
