I don't know of anywhere that you would be able to see these.

You need to think about the problems that you have completing each activity, why you have the problem and how it might help if you received some form of care or support.

So as an example; if you have Osteoarthritis in your hands then you might struggle to prepare and cook a meal, so you need assistance to be able to complete this task.

If you wanted to include this document in your appeal then you really should have sent it of TS last week!

Take a copy with you on Friday, better still six copies, and ask the Clerk of the Court to see whether the Judge will accept it or not.
