Problem is
Can prepare a meal but I cannot safely and repeatedly cook a meal, ie I cannot get close up to the cooker unless I get parallel to it in my wheelchair and as such my face is to close the heat source and splash, splatter and spill and burn level.
We have 2 front burners at almost eye level, so frying and boiling at eye level cannot be performed in a safe manner 365 days a year.

So you seem to have a good grasp of the issues and the criteria that you need to meet, that's what you need to put down on the form.
Think about aids that may enable you to cook, for example they will look at whether you could use a microwave.
That said bear in mind the following legal definition
“Cook” means heat food at or above waist height.
