Hi everyone,
I promised to update this post after I got my decision.
I filled the form in, checking the 'no change' box for each question as there hasn't been any change. I sent all of my original supporting evidence (from when I applied for PIP) and my social worker also wrote me a supporting letter.
I sent the form back on 26th July by special delivery (the deadline was August 1st)
I've received the decision today and I have been kept on the same award of enhanced for care and mobility. My original award was due to expire in July 2017, they have extended this until 2019.
I hope this might provide some reassurance for anyone else who received this new form.
