Yes, it is up to you to explain your limitations in relation to the ESA descriptors. Although this is a reassessment, you should treat it as you did your first claim. The ESA50 should be completed in detail and you should send in (copies of) any/all supporting documentation, even if DWP have had it previously. The assessor will have no prior knowledge of you, probably won't even know that you are already in receipt of ESA.

I am filling in my ESA form. In question 8, getting aroung safely, does this only concern people with sight problems or can I mention the difficulty I have because of my medical problems such as mobility and effects of medication?

I have extra pages to attach to my ESA form. Should I put my name at the top of each page and how should I attach it to the form?
Also, if my condition has not changed since I was put in the Support group 3 years ago should I get my G.P to say it has not changed since my last assessment.

Make sure that each page has your full name and NIN at the top and that they are numbered, we recommend "page X of Y" so that it is easy to see if pages are missing. Make sure you clearly mark on the form where your answers are and on the relevant page that you are answering question X.
Attach the pages to the back of the ESA50 using a staple or make a hole in the top left corner and use a treasury tag, string or ribbon to the form. The pages need to be easily detached so that they can be scanned by the DWP.
