First of all congratulations on the award you have received.
The main problem claimants with mental health problems have with mobility is the change of criteria between DLA and PIP, although they sound similar they are actually quite different.
To score higher you will need to show that you cannot go out on your own, you may be able to argue due to the need to do the activity reliably, that "cannot" does not mean "never", but the standard you will need to show will be far higher than the normal "majority of days".

Whilst entitlement should be automatic if you meet the eligibility criteria, you will have to contact the DWP to get them to look at it.
To be eligible for the SD Premium you need to be living alone, or considered to be living alone and no one must be claiming Cares Allowance against your PIP.
In addition, you must be receiving or entitled to receive ESA(IR).
So the first thing is to check whether your ESA is Income Related or not.
If it is then ask for the DWP office dealing with your ESA claim for an IS10 form.
If your ESA in not IR then you can check whether you are entitled by entering your information into one of the online benefit calculators.

You mother would be excluded because she is receiving DLA MRC, but if her carer is living with you then you are will not be considered to be living alone.
