There's no template they just need to explain why you cannot attend at their offices, the only thing you need to avoid is their saying that you would have problems travelling as this might result in the AP offering a taxi.

He printed out a generic page which had the consultation about my ears and that hen wrote in history, wants letter that cannot attend medical for social and benefits because of agoraphobia, anxiety and pain symptoms, agreed to give print out of consultation. Will need home visit for assessment.
I've been crying ever since I got home. The letter is written especially for Atos, please tell me what he wrote is ok? I explained really well it had to be a letter but he was running behind and very late. I just had to get out of there as I was feeling very sick and my son was hungry.
I might as well give up

It may be sufficient, the important thing is to get it to ATOS as quickly as possible.

I cannnot force the GP to write a letter to Atos. That's twice I've asked him for a letter. My previous GP was brilliant and would fax them for my ESA. But I had to change GP when I moved which is also when my health and disabilities went down hill. I have other mental health problems which they know about and they know these things trigger me. I've really had enough of it all.
