assessment report received - truly shocking

Having already registered my complaint about {removed] from Capita who did my PIP assessment, today I received a copy of her report. it is full of lies and I am truly distraught. I know I am not the only one who is suffering this indignity, but I feel utter and total despair. The lies start at the beginning with wrong time of arrival being documented and length of visit. A kindly response would not go amiss and is there anyone out there who can help me; where can I get help i just cannot cope.

I'm in a similar position, just received my decision and the reasons stated including things that supposedly happened at my assessment are just total fabrication. I didn't receive a copy of the assessment report just the actual decision. How can I get the report to understand where these comments are coming from?

I lose all my DLA enhanced rate care and mobility. Waiting for the decision letter, which has been sent out, but it was confirmed on the telephone today. It seems all my medical evidence from hospitals, GP that DWP had for consideration has been totally ignored. God knows what I am going to do with no care, especially as I am incontinent and cannot bathe myself. I truly am wondering whether it is worth carrying on; its bad enough trying to get through each day in constant pain without now having to 'fight' this. Phone the DWP and they should send out a copy of the assessor's report without any problem. We usually recommend following up any calls in writing so that you have an 'audit trail'.
