Have you had a Decision on your claim yet?
I'm afraid that it is not uncommon for claimants to have issues with their assessment report, although it tends to be indicative of the Decision, this is not always the case and DM's have been know to ignore the report and go with the information that the claimant has sent in.

Phone the DWP and they should send out a copy of the assessor's report without any problem. We usually recommend following up any calls in writing so that you have an 'audit trail'.
