Some areas are running at 6-8 weeks but others are running at 6-8 months or even longer, unfortunately we don't have information on which regions are affected in what way.

It will likely be an ESA113 asking your GP about your conditions and whether you are able to attend a face to face at their offices.
Speak to your HGP to see if they want to see you before they complete the form, some do and it is usually to your advantage to do this.

Hopefully it will be the one that you see regularly but some Practice's operate a policy of giving the form to whoever is available.
