Yes, it doesn't look good does it. I wonder if the GP is not very aware of doing these forms, or was juts in a rush. Not sure. And even if i go back and ask them to write it again it will still look bad as they will have the other copy as well. I only wrote the note to hurry along the GP, as Maximus said they were waiting for the info. Argh. Oh well, I suppose they cold have said I filled it in and it wasn't right, instead of saying they verify it, which means they agree with what I said. They also enclosed printouts of my conditions, meds and recent consultations so that is in agreement with my form at least. So I suppose in case of an appeal it is at least all consistent.
You need to put this in context, it's one piece of information, it may have a negative effect but it's not your whole claim and there is no reason at the moment to assume that you cannot receive the award you hope for.
