made a hash of ESA 50 claim form

Hello,

I received ESA 50 claim form , made a hash of it , so have printed off a full copy from the GOV UK web site. From the library computer and printer.

There is no number on the hashed DWP form, just asking return form by 12 th December.

It`s all black and white.

Will DWP accept this form ?
