If you are eligible for ESA(IR), despite your pension, then it will be paid as a top-up to your ESA(CB), not as a replacement for it.

The ESA3 is to see whether you are actually eligible for ESA(IR).

Would it make any difference to my claim if I was on middle rate mobility and lower rate care whilst on DLA?

No, neither of these have an impact on ESA.
