The form is a review form, a smaller questionnaire. A few things have changed for me which they need to know. My claim via tribunal was successful in March 2017. It was du for renewal next August so this came as a shock. The person helping me is telling me that they are not sending anything we used at the tribunal but I thought it would remind them perhaps as I doubt they are going to read last years questionnaire and all papers to compared to this short version which leaves a lot out. I just do not understand that they can stop payment or change it at a whim. Makes a mockery of my filling the forms out. Anyway thanks. Fingers crossed.
