The assessment report (PA4) is only supplied on request, contact the DWP office dealing with your claim.

Thank you for the info as no one had made me aware I had to request the PA4

I have notified them by phone I want an MR consideration as I not happy with the result.

Do I need to submit evidence and reasons now within 28 days ? As to why I disagree with the results.

You need to confirm that the DWP are allowing you 28 days to provide additional information but on the assumption that they are then you need to explain why you disagree with the Decision.
