I put in a request to my Daughters Doctor for a supporting letter 6 weeks ago, when it didn't arrive I asked for an extension, however the extension period is about to end, I am going to have to send in the PIP form without the evidence, shall I put a note with the form saying medical evidence will follow or any alternative advice would be appreciated.
