Yes I have still got the copy except for one page Q15 extra information. I am getting confused about how much to put down as the from is smaller and there have been changes especialy to the monitoring medication and treatments part. The envelope doesn't look big enought this time to include all the extra pages I sent last time.
any advice would help..... should i write the same amount or less?

Whilst I would encourage you to be brief in your answer do not let the constraints of the form prevent you from explaining your limitations for each activity, you can always use extra pages and your own envelope if necessary.

Hello Goordon, I was unsure as the return address is on the back of the form and the envelope has a window to show it. If it turns out that I can't fit it all in is it ok to just write the address on a larger envelope? EG PIP Payment 3 Wolverhampton post handling ite etc?
Also I noticed that the definition of Supervision" now means all of the time instead of part of the day. Does someone have to actually recieve 24hr Supervision or is it a case of "if you need Supervision but don't actally get it " that it is still counted that you need it as in the other activites od daily living?

Yes, it is perfectly OK for you to use your own envelope, just transfer the address from the form.
I think you have misunderstood "supervision"
First, you do not need to be receiving the supervision to score, you just need to show that you need it but you will also need to explain how you manage (or don't) if you are not receiving the supervision.
Secondly, supervision applies for the duration of the activity or task, not the whole day, so as an example, so as an example; if you need supervision while showering then it would the time of the shower that would count.

Thanks Gordon, I did misunderstand "supervision" So if B/f is unable to go out anywhere on his own because of anxiety, depression some suicidal thoughts and emotional flooding that means he is not able to concentrate or be safe around traffic and would be a risk to himself that would be supervision?
Also on another site I've just read that they suggest that in the box "Tell us about any changes or help you need etc" that you should write eg. ".I belive that I continue the following discriptor you need an aid or appliance etc awarding 2 points. This remains the same as my current award." ... I thought this was a bit odd as if you are telling them what to award? Would this be something that you would advise people to do? (haven't done it yet just want your thoughts and advice on it. Many thanks.

Sorry forgot to ask this bit The other site says send in the medical evidence you originally sent with the PIP claim form but am afraid we seem to have lost the letters we sent before and no longer see the cpn and have different gp now as old one retired. Surely they would already have seen this before? I thought you had to send new evidence if we can get any?
