Would it be best to note when explaining carpal tunnel symptoms that they can't predict outcome of carpal tunnel surgery due to other conditions & send in the proof

You have to put what you think is best. I say again - focus on the criteria and how you meet them. Only you know your difficulties and what impact they have. The best evidence you can give is where you have letters or reports from medical professionals confirming your diagnosis, symptoms and restrictions.
