When I include letter requesting paper based assessment along with Dis Affects You form, do I need to include med evidence to support my letter requesting paper based assessment?

The report is written entirely based on the information available to the assessor, your form, any evidence attached and any information that they have been able to source, for example; from your GP.

There is no interaction with the claimant, if that is deemed necessary then a telephone assessment will be arranged and the report will be produced from this and the other information.

Most claimants have medical evidence this may include a diagnosis but rarely indicates the limitations that may have been observed or could reasonably be expected to exist. The PIP assessment, Paper of otherwise looks at the functional restrictions that a claimant has in regard to a set of daily activities, so you need to find a way of bridging between your medical evidence and the problems that you have with those activities, our PIP Claim guide explains how to do this but you will need to provide your own unique spin on this as you complete the form.

Hi Hil

Yes supply as much supporting documents as you can to help with your claim, make sure you ask on the form for a paper based assessment and the reason why you need a paper based assessment, any supporting documents should also support your claim for a paper based review.

Thanks v much for this. Is a paper based assessment more or is it less likely to provide the material the assessor needs? ie does a paper based assessment have any advantages over a telephone assessment?

Hi Hil

The main advantage of a paper based assessment is that you do not have to talk to an assessor, it is purely based on your submission, it can also be a disadvantage as you are not able to explain your disabilities and how they affect you on a day to day bases.

Telephone assessments have taken the place of face to face assessments for the majority of claimants, the advantage and disadvantage are as above.
