Is there a template or suggested wording for requesting a paper based assessment? Is it necessary to contact DWP to check they received my request? Thanks.

Hi Hil
No there isn't a template for requesting a paper-based assessment - you just need to ask, but you need to say why and as Gary said in an earlier post if you have a letter supporting your request for a paper-based assessment that increases your chances of getting one.

Hi Hil, I have just succeeded in getting a very good result for daughter via paper based assessment. I first asked for it by phone when a text out of the blue to her with a phone assessment appt caused her a meltdown - there wasn't time to write. The GP then wrote and the key phrase is that a phone or face to face assessment will cause overwhelming psychological distress [OPD was used as an abbreviation in the assessor's report]. Unless of course it is something like speech difficulties or breathing problems that prevent you doing phone well. But see also today's news item on here about having someone else 'do' the phone assessment for you.. Best wishes, Denby.
