Hi,
I am using the excellent Best Possible PIP Guide’ (version 40 Aug 2020) and have a Q about supporting evidence:

Page 71 (Supporting Evidence) bullet point n. six ‘Telephone conversations with any such professionals’
What constitutes evidence for these telephone conversations? Is the health professional’s name, work title, date and the main topic sufficient or are specific details best?

You can give a general topic of conversation - details are sensitive to you and you do not have to go into minute detail.

Just to confirm, the evidence being discussed in the Claim guide is that being collected by the assessor or DWP, it is not referring to evidence that you may have collected or conversations that you may have had with an assessor.

If such a conversation happens then it should be documented, I can't remember the form name and included in the evidence made available to the Decision Maker.
