As I said in my previous post, the section of the Guide that you are referring to is about assessors collecting evidence not you doing so.
You can include the details of conversations that you have had but I would not rely on them helping you with your claim, they will not substitute for the Health Professional saying that they have spoken to you and doing so in writing.
