If i request a copy of my medical records from my GP's surgery will that include all the notes he has written down from the times i have spoken to him over phone over during the past few months? also if i do request copies will my GP know i have made a request if i addressee the letter to the practice manager?

They should but if your GP is anything like mine then the notes may not be that detailed.

Just to say that I recently requested copy of my records. Like previously I wrote a letter. I think perhaps now that we don't have to pay as it's done via FOI (Freedom of Information), the secretary had to send me a form to complete, which I then had to return.
So, in case you and others' are against the clock, might be worth allowing extra time for form to be sent and returned!
